This is a medical examination suitable for applications for Employment, Dependent, LTV, Student and S Pass. This Package covers: - Physical examination by a doctor - Height, Weight, BMI - Chest X-Ray** Note: (**) - Available at a partner imaging centre, our clinic staff will assist with appointment booking during your clinic visit

Service Description

Important Notice:

1) Kindly refer to the letter from MOM to select the correct screening package.

2) As Chest X-ray is required, it will be conducted off-site, our staff will assist with the booking during your visit.

3) Results will be available for collection at the clinic within 7-10 working days.

4) Rescheduling of appointment is only allowed up to 2 working days before the scheduled appointment date.

Identity card (Pass, Work Permit)/ Passport

Screening Form (if applicable)

Recommended For:

For first-time applications for work and study pass

Pre-screening Preparations:

This package is not recommended if you experienced fever symptoms 3 days prior to your appointment.

Terms & Conditions:

• Kindly note that appointments for purchased health services online must be scheduled and fulfilled within three (3) months from the date of purchase.

• Failure to schedule or complete the appointment within this timeframe may result in forfeiture of the service without eligibility for a refund.

Cancellation Policy:

1) Unfortunately, no cancellations or refunds are allowed. However, if required, screening can be postponed to a later date (with a validity period of 6 months from the time of booking).
