Recommended dosing:
● Booster shots are recommended every five years thereafter if the risk persists.

Not recommended for:
● Persons experiencing fever symptoms 3 days prior to appointment.

Documents required for the appointment:
● Parent's Identity card
● Child's Birth Certificate (if applicable)
● Child’s Health Booklet (if applicable)

Rescheduling and Cancellation Policy:
● Cancellations are not allowed once payment has been made. However, if required, appointments
can be postponed to a later date (with a validity period of three (3) months from the time of
booking).

For Confirmed Appointments:
● For appointment rescheduling less than 2 working days before the confirmed appointment date.
● You may reschedule your appointment up to a maximum of 2 times.
