Service Overview
LPA is a legal document which allows a person who is at least 21 years of age ('Donor'), to voluntarily appoint one or more persons ('Donee(s)') to make decisions and act on his/her behalf if he/she loses mental capacity one day. Donor will require a Certified Issuer (CI) to witness and certify the LPA before submission.
DA offers a seamless service from the convenience of your home to witness and certify the signing of LPA.
Service Description
Preparation:
Please ensure that 1) you have completed your LPA online via OPGO and that 2) both you and donee are available on the selected date. Do also remember to bring your mobile phones with the Singpass app installed for digital signing.
Terms & Conditions:
- Kindly note that appointments for purchased health services online must be scheduled and fulfilled within three (3) months from the date of purchase.
- Failure to schedule or complete the appointment within this timeframe may result in forfeiture of the service without eligibility for a refund.
- Doctor Anywhere reserves the right to update the information or fees associated with this service without prior notice.
Cancellation Policy:
- We do not accept any cancellations / refunds except in the following scenarios:
- The patient is medically unable to complete the service.
- System errors resulting in duplicated orders or order errors.
- DA is unable to provide the service purchased.
- Any cancellation request must be made within 7 days from the date of purchase and at least 3 days prior to the appointment date.
- Once the service has commenced, cancellations or refunds are not allowed, except under special circumstances.
- Please note that DA Marketplace does not allow for any cancellations due to change of mind.
