Please ensure that the witness is an independent individual who is not a beneficiary, the spouse of a beneficiary, or named in any related insurance policy. Appropriate witnesses may include friends or extended family members who are not immediate relatives.

Terms & Conditions:

• Kindly note that appointments for purchased health services online must be scheduled and fulfilled within three (3) months from the date of purchase.
• Failure to schedule or complete the appointment within this timeframe may result in forfeiture of the service without eligibility for a refund.
• Doctor Anywhere reserves the right to update the information or fees associated with this service without prior notice.

Cancellation Policy:

• We do not accept any cancellations / refunds except in the following scenarios:
  • The patient is medically unable to complete the service.
  • System errors resulting in duplicated orders or order errors.
  • DA is unable to provide the service purchased.
• Any cancellation request must be made within 7 days from the date of purchase and at least 3 days prior to the appointment date.
• Once the service has commenced, cancellations or refunds are not allowed, except under special circumstances.
• Please note that DA Marketplace does not allow for any cancellations due to change of mind.
