Important Notice:

Any add-on imaging tests will not be conducted at our DA GP clinics. They will be conducted at our executive health screening centre (DA MedSuites) or our partner radiology clinics. Don’t worry - our staff will assist and advise accordingly during your visit.

What happens after your purchase:

• After completing your purchase, you’ll receive a confirmation email and purchase receipt for your records.
• The gift voucher will be mailed to the address provided within 3–5 working days.
• Once received, the recipient of the gift voucher can simply scan the QR code to select their preferred location (if applicable), date, and time for the appointment.

Preparations for the person utilising the gift voucher:

• Please bring along the physical copy of the voucher and your identification card (physical/digital) on the day of your appointment
• If you are experiencing fever symptoms closer to your appointment date (< 3 Days), we kindly request you to reschedule your appointment by following the rescheduling instructions sent in our appointment confirmation email
• Please avoid strenuous activities for at least 24 hours before your appointment
• For women, urine tests should be done at least 5 days after the last day of menstruation

Terms & Conditions:

• This voucher is valid until the expiry date as stated herein.
• All appointments must be scheduled and completed within the validity period. Any unused services after the expiry date will be forfeited without refund or extension.
• Any request to reschedule confirmed appointments must be made in accordance with Doctor Anywhere’s appointment policy.
• This voucher is for single use only and is not exchangeable for cash, credit or other services, and cannot be replaced, refunded or extended if lost, damaged or expired.
• This voucher must be presented on the appointment date for redemption. Only original vouchers will be accepted.
• This voucher covers only the health screening package specified herein. Any optional add-on tests, services or consultations may incur separate additional charges.
• Doctor Anywhere reserves the right to amend the terms, conditions, pricing or screening package components or associated services at any time without prior notice.
• Doctor Anywhere may, at its sole discretion, determine a patient's suitability for health screening services based on your physical condition and reserves the right to decline provision of all or any portion of such services if deemed medically unfit.
• Doctor Anywhere shall not be liable for any consequential, special, indirect, incidental or punitive damages, losses or expenses and for the quality, accuracy or outcome of the health screening services.
